Hi, for one of our apps, which has not been updated for a while, we have received a policy violation notice:

Rewarded Ads - Disallowed Rewarded Implementation

Rewarded ads may only be served after a user affirmatively opts to view a rewarded ad. Additionally, a user must opt to view each rewarded ad individually and cannot opt-in to automatically seeing rewarded ads.

The key information here is that the app does not have any rewarded placements. We had ad placement in Admob dashboard for rewarded but without any ad network connected. I have removed the unused rewarded ad placement in the configuration panel and asked for a review. It was again rejected. What I am doing wrong here?
