With regard to your concern, no ads fill is expected if the location you are in has a small amount of ad inventory, then when you tried using VPN it may connected to other location who has large amount of ad inventory. Please start a new thread if your issue is not related to using a VPN. Then our team can diagnose your particular case.
Before you do though, as a first step you can try enabling test devices while still using your real ad unit.
