Use custom open-elevation server

I use a self hosted open-elevation server, so I made small changes to FixElevation.cpp and Settings.h to be able to use a custom url instead of the hardcoded url. By default it uses the open-elevation public api url.
If this is helpful for others too, I'm happy to make a pull request on Github.

If you want to make a PR see https://github.com/GoldenCheetah/GoldenCheetah/wiki/Guidelines-for-submitting-a-patch,
and try to explain the rationale, it is to clear to me the usefulness or how common can be somebody taking to go this route not be able to build by him/her self.

I can see this being useful because ...

a) open-elevation.com is often overloaded so the fix elevation operation fails.

b) I have access to much better DTMs for my locale 

c) it's encouraged and supported by open-elevation.com
