Portuguese translation assistance

Would anyone help me to translate 4 lines of a 200 plus year old baptism record. Someone well versed in Portuguese should be able to do this quite easily. I promise I won't make a habit of this but since it is only four lines, I don't feel like hiring a translator :-)
