Details:

The attached patches modifies three -me-related documentation files.

doc/meref.me: clarified the behavior of the .$H macro, explaining that in
addition to the top of the page, it gets run at the top of each column in
two-column mode.

doc/meintro.me and doc/meintro_fr.me: removed a paragraph that probably hasn't
been true since 1986, and certainly isn't today.
