I often have a list of items, and like it to look like this:

\item bla bla bla bla bla bla bla bla bla bla bla bla
bla bla bla bla bla bla bla bla bla bla bla bla
bla bla bla bla

etc. I write it like that, and as I type in more lines, they get
nicely margined in the same way.

However, if I now change the text, and then reformat using ESC-q,
it changes to

\item bla bla bla bla bla bla bla bla bla bla bla bla
bla bla bla bla bla bla bla bla bla bla bla bla
bla bla bla bla

Can I force it to keep my chosen margins?

I assume you use AUCTeX for LaTeX editing, right? If so, you could set

(setq LaTeX-indent-level 6)
(setq LaTeX-item-indent -6)

and it should fill \item paragraphs as you like. However, now every
environment's content will be indented with 6 instead of 2 spaces, too.
