Unfortunately C-= does not work in emacs under most terminals unless you
customize them. My suggestion is leaving both bindings in place and
remove the deprecation warning.

Hi Wraithan!
This is fixed by an additional binding to C-c C-a C-a in official
release V1.3.1. Latest version at
http://www.merten-home.de/FreeSoftware/rst_el/ .
This is in the SVN revision r7490 upstream in the Docutils project
(see
http://docutils.svn.sourceforge.net/viewvc/docutils/trunk/docutils/tools/editors/emacs/)
and r109303 in Emacs Bazaar (see
http://docutils.svn.sourceforge.net/viewvc/docutils/trunk/docutils/tools/editors/emacs/).
