Thanks Becky - man, that's quite a handbook - lots of info!
I haven't been through it thoroughly yet, but you might consider zero spacing the Table of Contents - right now it's a bit difficult to find what I'm looking for.  More to come - hopefully - great job!

 Wow, what a lot of info!   The only suggestion I can think of is maybe an Appendix with those great charts about what is expected of a QA person in each role at each stage of a project.   That way, someone can just quickly go to the back to see just those cheat sheets.   I'm guessing that what most people will want -- the quick cheat sheets.   But, I wouldn't want to create a situation where the information in two sections contradicts each other as the document evolves.
