You should only submit the forms listed below for special situations. 1. Reporting Your Passport Lost or Stolen (DS-64)
- You should complete this form if you lost your passport or it has been stolen. To replace a lost or stolen passport, you must apply in person and submit Form DS-11 in addition to Form DS-64.

In addition to generating a completed application form, this website also creates a barcode that is placed at the top of the generated passport application form. This barcode contains the information gathered from the web page, and is later used to easily read in and process the information on your application. For the barcode to be generated correctly, all required fields must be completed at the time the form is submitted for creation.
If you fail to provide the information, you will encounter a delay in processing and/or denial of your passport application. You will also be subject to a $500 penalty enforced by the IRS pursuant to Section 6039E of the Internal Revenue Code (26 U.S.C. 6039E). I went to the effort to complete the on-line form DS-82 from the US State Department. It was easy to fill in. I am then to print the form and then mail it in. However, Adobe restricts printing! Error message says "Printing is not permitted as the document has restricted printing permissions." Is there any way around this? Why would the government have a user fill in a form if the form could not be printed? (Yes, I know it is our government.....)
I had no problems printing, but as I recall, it was an odd process. You have to fill out the form completely and when you are finished it makes the PDF, which is printable. You can't leave any blanks, and I had to go look for some of the info. It is not a PDF until you complete the process.

In case the existing OCI card holder wishes to update the latest information about his latest passport, Change in address/Occupation/Contact details, the OCI card holder can select the option as given below and upload the latest photograph and requisite documents. These details shall be updated into the OCI system records and an acknowledgement shall be shared with OCI card holder on his/her registered email ID. In case the existing OCI card holder wishes to apply for re-issuance of OCI Card, application form, complete in all respects, along with all relevant enclosures and requisite fee may be submitted to the Indian Mission/Post/Office in whose jurisdiction the applicant is ordinarily residing irrespective of the fact whether the same Indian Mission/Post/Office has issued the original OCI documents or not.
