If a student has suffered a possible concussion, please seek medical attention. Have the Doctor complete and sign the concussion forms listed below (or have the doctor complete their own forms) and return them to the school nurse. If the student is diagnosed with a concussion, the student should stay home from school for 24 hours after the injury and they then may return to school if feeling better. It is recommended that the student attend school for half-days for the first few days back at school as needed. The nurse will communicate with teachers and staff about the Doctor recommended accommodations during the recovery.
An Apostille is a certification provided under the Hague convention of 1961 for authenticating documents for use in foreign countries. An Exemplified certification covers those countries who did not sign the Hague Treaty. The sole function of the Apostille or Exemplified certificate is to certify the authenticity of the signature of the document. In Florida, the Apostille or Exemplified certificate contains the original signature of the State Registrar and the Secretary of State. A Florida Birth certificate can be certified as Apostille or Exemplified by the Florida Department of State.
The State of Florida issues commemorative birth and marriage certificates, signed by the current Governor and State Registrar of Vital Statistics. The certificates contain calligraphy style printing, gold embossed state seals, and are suitable for framing and preserving as family heirlooms, but not to be intended as proof of birth. The commemorative certificates are mailed encased in cardboard shields to ensure protection.
In 2006, Florida's governor signed into law legislation that allows for the issuance of a certification for a Certificate of Birth Resulting in Stillbirth. This law, section 382.0085, Florida Statutes, is known as Katherine's Law. Florida was the 14th state to implement such a law.
The Certificate of Birth Resulting in Stillbirth is not proof of live birth and may not be used to establish identity. Gestation must be 20 weeks or more, and there must be a fetal death certificate on file with the Florida Bureau of Vital Statistics in order for a stillbirth certificate to be prepared. The information included on the stillbirth certificate comes from the fetal death certificate.
Lee County does not prepare elevation certificates. Elevation Certificates (EC) are prepared by locally licensed surveyors and submitted to Lee County Community Development as a requirement of the building permit process. Once submitted, Lee County keeps a copy of the certificate on file (see below for information on searching for an elevation certificate). You will need to contact a locally licensed surveyor if we do not have one on file and you need a certificate prepared.
However, there are no elevation certificates for structures built before 1984 or on parcels that were designated in the floodplain for the first time in August 2008. Those structures did not require elevation certificates because they were not in the floodplain at the time of construction.
Effective Jan. 1, 2017, a copy of each elevation certificate prepared by a surveyor and mapper will be submitted to the Florida Division of Emergency Management (FDEM) as required by Florida Statute 472.0366. FDEM worked with representatives from the Florida Surveying and Mapping Society, the Florida Board of Professional Surveyors and Mappers, and the office of Florida Senator Jeff Brandes to create a solution to meet the requirements of the law. Bonita Shviraga is Adjunct Professor at the Midwifery Institute at Jefferson University. She obtained her nursing and nurse-midwifery education at the University of Illinois Medical Center Chicago and obtained her PhD in nursing from the University of Colorado Denver. Her ethnographic research investigated exclusive breastfeeding in recently immigrated Mexican primiparas. She has worked in high-risk inner city tertiary care centers, a hospital birth center, community clinics and hospitals, and community health. She practiced full scope midwifery for more than 30 years in various public and private settings serving persons from diverse backgrounds. As a faculty member she has taught in interdisciplinary educational settings with nursing, nurse-midwifery, nurse-practitioner, physician assistant, and medical and osteopathic students. Within an interdisciplinary faculty model she taught obstetrical and family practice residents. Coursework she has taught includes Intrapartum, Postpartum/Family Planning, High Risk Obstetrics, Research, and Theoretical Foundations of Midwifery. She was lead midwife and then Practice Manager in the practice where she worked for 20 years. She has served on various hospital committees including quality, peer review, credentialing. She was also the Chair of the Magnet hospital Research Committee. Service work includes serving on the ACNM Certification Commission as Midwifery Certification Exam Chair, and as a member of the Board of Review of the Accreditation Commission for Midwifery Education. She currently serves as the Denver Regional Nightingale Association Co-Chair. Internationally she has done health volunteer work in Guatemala, Mexico, Belize, and Fiji including teaching Helping Babies Breathe nationally and internationally. As a midwife, Bonita embraces partnering with patients in their care and supporting physiological birth. As a faculty member she enjoys partnering with students and supporting them to achieve their goals while enjoying the journey. In her free time Bonita enjoys reading, hiking, travel, and spending time with friends and family.
Dr. Jernberg grew passionate about endodontics and decided to pursue residency. Dr. Jernberg graduated from residency and received his Endodontic certificate from the University of Nebraska Medical Center and is a board eligible endodontist.
