How to create Geoodk CSV file format into shape format

Looks like you were using the wrong converter. It can be a bit confusing

When you create a form you convert the excel sheet (your xlsform) to xml with 

http://geoodk.com/xlsform_converter.php (the first upload)

Then the GeoConverter is for converting to shape you use  http://geoodk.com/xlsgeoconverter.php (the first upload)
