If a sequence exists with
a character linked
the character with an origin universe selected
the character with a member-of-team selected
the team with no origin universe selected
and you edit any part of the sequence (say, the story title), the team is automatically edited to add the character's origin universe, but this results in the team being listed twice: once with an origin universe and once with no universe (and no members, since their member-of-team origin universes have all been auto-updated to a universe).
If you have the sequence editor set for tabs, you won't even see this change. It does show up in preview, but the behavior should be to delete the team with no universe and no team members when the characters are auto-updated.

This is due to migration aspects and a documented behavior. The behavior will disappear once the existing sequences are touched.

But touching the existing sequence yields two groups, one with a universe and members, and one with no universe and no members.

yes, touching will do that and needs editing.

This only affects migrated entries, it won't affect newly added entries, so at some time this will not occur any more.
