As of January 1, 2014, applicants can apply for Child Health Plus coverage through the NY State of Health: The Official Health Plan Marketplace. Paper, fax, or email applications for Child Health Plus will not be accepted by Child Health Plus Health Plans after December 31, 2013.

New York State has a health insurance plan for kids, called Child Health Plus. Depending on your family's income, your child may be eligible to join either Children's Medicaid or Child Health Plus. Both Children's Medicaid and Child Health Plus are available through dozens of providers throughout the state.

Protecting and improving the health of children is of fundamental importance. Over the past several decades, we have seen dramatic progress in improving the health and reducing the mortality rate of young children. Among other encouraging statistics, the number of children dying before the age of 5 was halved from 2000 to 2017, and more mothers and children are surviving today than ever before.

However, a great deal of work remains to further improve the health outcomes for children. The world is facing a double mandate. More than half of child deaths are due to conditions that could be easily prevented or treated given access to health care and improvements to their quality of life.

At the same time, children must also be given a stable environment in which to thrive, including good health and nutrition, protection from threats and access to opportunities to learn and grow. Investing in children is one of the most important things a society can do to build a better future.

WHO works with partners and Member States to improve the lives of all children and meet the targets of Sustainable Development Goal 3, which aims to end preventable deaths of newborns and children under 5 by 2030. If all countries reach this goal, 11 million lives will be saved, more than half of them in sub-Saharan Africa.

Child Health utilizes a Patient Centered Medical Home model. A medical home is a family-centered approach designed to address interrelated medical, social, developmental, behavioral, educational, and financial needs to achieve optimal health and wellness for children and their families. We can assist you with specialist coordination, housing resources, transportation, medical supplies, behavioral and developmental referrals, as well as financial resources.

Students are exposed to cutting-edge pediatric clinical care at multiple teaching hospitals including Phoenix Children's, one of the largest freestanding children's hospitals in the United States.

They receive training from a large and diverse group of board-certified pediatric specialty and subspecialty educators. Instruction is provided in both hospital and outpatient pediatric settings and is integrated into the educational structure of a large pediatric residency and multiple pediatric fellowship training programs.

A rich pediatric research environment has been created from Phoenix's leading scientific and medical institutions. The goal is to advance discovery, facilitate translation and accelerate clinical application.

Making sure your child is physically healthy is an important part of supporting their ongoing growth and development. Child physical health is more than just tracking their height and weight. It also includes making sure they have good nutrition, enough physical activity, restful sleep, timely immunizations, and strong dental health.

Proper nutrition lays the foundation for good health. Encourage healthy eating by offering your child nutritious meals, snacks, and beverages to fuel their growing bodies. Making nutritious choices for your child promotes healthy growth and helps your child develop good eating habits for life.

Breastfeeding is an excellent source of nutrition for newborns. AAP recommends breastfeeding as the sole source of nutrition for your baby for about 6 months. But breastfeeding can be hard and in certain situations, it may not be possible. If it is not possible to feed your baby breast milk, infant formula will still provide balanced nutrition. Continue breast milk or infant formula until your baby is at least 1 year old.

Dental health is important for children of all ages. Good dental health prevents tooth decay and other issues that may cause mouth pain when children eat and speak. You can promote good dental hygiene by doing the following activities:

We collaborate with policymakers, providers, and partners to transform child-serving systems, disseminate evidence-based and best practices, and advance policy solutions that result in better outcomes for children in Connecticut and beyond.

You may also have to pay small co-pays when you or your child receives CHP+ services. There are no co-pays for preventative care, such as prenatal care and check-ups. Other services may require co-pays that vary based on your income. Native Americans and Alaskan Natives do not have to pay co-pays. Contact your Managed Care Organization (MCO) for their applicable copays.

Each year, the maximum amount of money (referred to as out-of-pocket limit) that you are required to spend on health care payments for all members of your household that are enrolled in CHP+ is 5% of your annual income. If you are found eligible, your coverage will last for 12 months after your pregnancy ends. Coverage will end 365 days after your pregnancy ended. Members need to report their pregnancy in order to receive postpartum coverage.

DentaQuest provides dental benefits to all eligible and enrolled CHP+ members. Benefits include preventive and diagnostic services, restorative services, endodontic, periodontic, prosthodontic, oral surgery, and limited orthodontic services. See our CHP+ Dental Care page for more information.

CHP+ members are enrolled into a Managed Care Organization (MCO). An MCO is a group of doctors, clinics, hospitals, pharmacies, and other providers who work together to help meet your health care needs. Each CHP+ MCO uses its own group of hospitals, pharmacies and doctors for the counties it serves.

The county you live in will determine which MCO you or your child enrolls with. If more than one MCO is available in your county, a health plan will be selected for you, but you can change MCOs for up to 90 days after you qualify for CHP+.

I love CHA and have been coming here since 2015. I have two sons, 7 yo and 18 months. I primarily see Dr. Shafer and Dr. Lautner, but have seen all doctors at some point. Whenever my children have been sick, I've gotten a fast appointment without long wait times. All staff is extremely courteous and professional, I can honestly say within 7+ years, I've never had a complaint. One thing to note - one of the main reasons I chose this practice is because they are pro-vaccination. If you are planning on skipping or delaying vaccines, this most likely is not the practice for you.

I always have a great experience here. I drive a decent distance to get here and have been bringing my kids for 6 years. I've seen almost every doctor and they're all wonderful, especially Dr. Schafer and Dr. Koval. The doctors and all the staff are so great with my kids and I always get right in and seen as soon as we arrive. I don't think I've ever waited longer than 5 minutes. They're very communicative and the nurses usually call back within a half hour when I call for advice. They're not pushy and are open to hearing different points of view. For example, I had heard from other moms that pediatricians aren't very open to Baby Led Weaning, but when I brought it up here, they asked me to explain how it works and said it seems great and as long as I'm doing it safely, if it works for me then I should do it. I'm always very pleased when I walk out and would recommend them to anyone looking.
