Urgent Help

Hi guys,

I need an urgent help. I received an email of admit on the 25th where
they asked me to mail back acceptance form (which they have not
attached). Contacted them via email on multiple occasions and still no
response. This form has to reach them by the 8th I am told. If someone
can forward me a copy of this acceptance form (fingers crossed and
hoping it is generic), would be very grateful.

Oh and I need the details of the mailing address where I am supposed
to send the financial documents. Would be grateful if someone can post
the same, because I am planning to send out the financial documents
today :)

There was no acceptance form for me. I had to email the graduate
coordinator saying that I was coming. Maybe they have different
procedures for different departments. Ur best bet would be to call
them, i've done that so many time since they dont reply to emails.

I finally got the hard copy yesterday. :)
