Manual upgrade from 3.1 Revival to 3.1.5?

Started in 2. All sorts of issues upgrading to 3 and then 3.1. Current version works (home page sez 3.1.0 Revival, but I am not sure). Custom theme. Need to upgrade to work on latest PHP.

Is it possible just to do a file by file comparison and manual update to 3.1.5 rather than risk the auto upgrader?

Or are there database changes and other stuff under the hood that the upgrader handles?

That's how I've done most of my upgrades. I have way too many modifications to core files to have them over-written :)
