Hi Rahmah,
I will try and get your application rushed, but I have no control over the embassies. Here is the information for the application.
The following calculations are based on a October 1, 2014 arrival date, but we have to start the funding from September 1, 2014 in order to issue a Ds2019 earlier than October 1st.
J-2 Dependent Application
To bring your dependents you need to look at this link and follow the instructions:
http://foreign.fulbrightonline.org/current-fulbrighters/in-program-requirements/bringing-your-family
In order for your family members to apply for J-2 visas they must apply directly at the U.S. Embassy in their home country. Among the documents required for a J-2 visa application is a DS-2019 form from IIE. In order to obtain the DS-2019 form from IIE, submit
to me the following documents:
1. A fully completed and signed
J-2 Dependent Data Sheet (see attached).
The returned form must be completed and signed.
2. Proof of funding for your J-2 dependents. This can be proven with either a recent bank statement or letter from your bank verifying sufficient funding that includes your name, account number, name of bank, current date and account balance. You have to provide
proof of funding in the amount of $32,040.00USD which is 100% of your monthly stipend amount for length of your Ds2019 which is two years long. This is $667.50 per month for 2 years for your husband and then $333.75 per month for 2 years for each of
your children. If you need to use a guarantor I’ve attached a letter you can use along with their bank statement, but you have to prove that you have a total of $32,040.00USD. The bank statements can be from anywhere in the world and in any currency.
3. Verification of health insurance coverage for all 3 of your dependents. They will not be covered under ASPE.
4. The biographical data page of each dependent's passport (including full name, date of birth, and place of birth).
This application should be scanned and emailed to me. It will take IIE 4 weeks to process the application. Once it’s approved we will send the Ds2019s to you
in the US. Then once they are here I need a scanned copy of their Ds2019, I-94 card and J-2 visas.
Please let me know if you have questions.
Also you haven’t completed steps 2 and 3 from my first email. These are also required for us to process the J-2 application.
2. Submit a Health Information Release Form
Attached to this email is the Health Information Release form. You’ll see you need to print the
Grantee Authorization for Release of Health Related Information, then sign it, scan it and then upload it to the Portal:
https://mycusthelp.info/IIE/_cs/Login.aspx?sSessionID=&target=%2fIIE%2f_cs%2fCustomerIssues.aspx
3. Submit a Proof of Full Time Enrollment
Next you will need to submit your official proof of enrollment once you start your classes. Fulbright requires this document to be an official copy from the Registrar’s office. So please go to the Registrar’s
Office on-campus and ask for one. Once you have it please log back onto the Portal and upload it. Instructions on how to upload it are attached.

Hi Rahmah,
Thanks for your email. How many family members do you want to come? I need to know to give you information on how much funding you need to provide.
I also want to let you know that normally the application process takes 1 month and then you also have to consider the time it will take to get the
visas at the US Embassy- this could take 2 months to complete.
Best,
Jacqueline

Subject: DS2019s for Family
Dear Ms. Sindoni,
I have reported my arrival to IIE through the Fulbright Self Service Portal and will submit all other documents as you instructed soon. I have moved in to my on-campus family housing and registered to my classes for this fall semester. Everything runs pretty
well for me here in UNM.
Forwarded is the email I received from AMINEF saying their approval to bring my family earlier to the US because my second child is still breastfeeding. I was told to contact you about the DS2019s for family once I arrive in Albuquerque. Thus, I would like
to ask for your help providing me the DS2019s needed for my family in order to process their visas. Thank you.
Best Regards,
Rahmah

Dear ibu Rahmah,
We have discussed your case in the office and we would make an exception considering that you are still breastfeeding your baby. Adeline is going to contact you regarding the details, the documents you must submit as soon as possible.
Please keep this to yourself and don’t discuss this with the other grantees because the decision is made on an exceptional case by case basis.
Regards,
Piet

Subject: Proposal to Bring Family to the US
Dear Pak Piet,
I am Rahmah Fithriani, a Fulbright Grantee of Presidential Ph.D. Program. I will start my study at the University of New Mexico in the beginning of August. Considering the duration of my study which can be 5-6 years, I plan to bring my family to the US.
I have two daughters, the first one was born on December 9th, 2011 and the second on September 26th, 2013. Since my second child still breastfeeds, I would like to ask for the permission from AMINEF to bring my family earlier than the time set by AMINEF
(after 1 semester). If it is possible, I want to bring them within two months after my departure. Two months will be the maximum time I can stock my breast-milk for my baby when I am away. I believe that the presence of my family will not be a problem for
me in adjusting with my studies and campus life since my husband will be at home taking care of them while I am attending my classes.
I do hope that this proposal be granted. Thank you very much for any helps given to me.
