I recently attended the Human Factors Analysis and Classification System (HFACS) workshop in Dallas. HFACS is a system that has been used by the Navy and Marine Corps for mishap analysis and investigation for many years, and has been adopted by many high reliability other organizations.  HFACS "bridges the gap between theory and practice to effectively integrate human performance and system safety principles and tools into your organization."

If you are looking for tools to put in the SMS toolbox, this is one that you can really use. The next workshop is in Las Vegas, Dec 8-10 and there are a few slots left open.
