Shortcuts in v4.0.5

I have assigned a shortcut in Edit>Preferences>Shortcut to a keystroke and it was working for a long time but then i created an Extension and ever since then my custom Shortcut keystroke stopped working and the keystroke does not show up in the list of Shortcuts anymore.  

Basically I wanted the F6 key to do a Tools>Code Formatting>Re-Indent so I added a custom shortcut in Shortcuts but after Apply/OK the keystroke does not do anything and when I go back and check the setting it is no longer there.

I mention about introducing an Extension because I vaguely recall reading somewhere (I'm new to this group) that somehow Extensions might cause this behavior.

Is this a known situation?  Is there something I should do?

PS. You made my day :-) 
I kicked out the extension (preferences ) : a file browser). 
And it works again! (shortcuts)
