Subject: COVID-19 Vaccine information
Hello everyone. As you are all aware CHOP is in the process of offering vaccines in phases to all employees including graduate students and part time staff. Everyone will need to complete a pre-COVID survey in My Health (same place as the flu shot surveys) prior to your appointment if you are choosing to receive the vaccine . You can also complete this prior to getting your appt. invitation.
To access this you need to log in to your My Health Account https://chop.readysetsecure.com/rs/#/myhealth/welcome located on the Occupational Health intranet site and complete the pre-COVID 19 questionnaire.
I know some staff have already received notification to schedule their first dose and rest assured that ALL CHOP employees will be offered the vaccine according to the attached time line distributed earlier this week. Phasing was determined by job role, with the majority of CCMT based staff placed in Phase 2 and Phase 3. If you have not received an invitation to schedule yet, you will soon. If you do not receive an invitation to schedule by 1/23, or have questions, please contact me or Occupational Health at OHC...@email.chop.edu for help.
This is also a good time to remind everyone to make sure that your contact information is accurate in Workday. You will receive both a text message and an email notification with the link to schedule your first dose appointment at one of the 3 currently available locations (CHOP Main, Buerger or King of Prussia).
