II. Employee chose to initiate travel AFTER the CHOP travel restrictions or advisories were
communicated. Employee is now required to contact Coronavirus Hotline at 800-722-7112, and press
1 to report your travel, and receive guidance from trained staff. Employee is now required to selfisolate
for up to 14 calendar days from the date of return from travel. For location-critical staff, a
shorter duration of 7 days of quarantine may be available, subject to ongoing symptom monitoring
upon return to work.
A. If they can work from home with manager’s approval they will be considered in an
alternative work arrangement (AWA). No PPL will be required.
B. If they are in a role that does not lend itself to working remotely, the employee will use
available PPL, take this time unpaid or utilize the limited advance of PPL described below.
C. If at some point during the 14-day period, the employee becomes symptomatic, tests
positive and remains ill, they will transition to Family Medical Leave (FMLA) and short-term
disability coverage to the extent eligible under the applicable policies and/or plan documents.
