Hi all.  Grad students and NTPs will need to create an account in order to complete the pre-COVID19 vaccine questionnaire.  This is not necessary to schedule a vaccine appointment but is needed prior to vaccine receipt. 

 

Attached are the instructions on how to do so.  Can you all please pass this along to your grad students?
