With the new high deductible insurance policies, it seems that a
larger and larger part of my job has become sending invoices directly
to patients.

I have poked around with "create statement", but find that the
invoices are PDF's and are not editable. Am I missing another way to
create invoices, perhaps something that will let me look at what I'm
prepared to bill, then make changes (draft invoices)? Is there a way
to create editable invoices? Add company logo and sheet setup (I'm
guessing that would be editing an xml file)?

Simple answer: not in the 0.8.x series of FreeMED.

Longer answer: we have moved to a much richer reporting engine in the
0.9.x series which is currently in trunk, so it's definitely possible
to do nicer formatting things with reports. Prepping statements is
something that is being handled in the ledger code, but we can flesh
it out more if there's interest.

TBH,

FreeMED 0.8.x has an alternative way to generate patient statements
with Agata report.
Key here is to modify patient_statement.report file located in /usr/
share/freemed/data/report

Look for Sample_Statement.pdf in the file section for an example of
what can be done.
I am printing these statements on the special pre-cut paper that
allows easily detach 1/3 top portion of the page.
It formatted for double window style envelopes, so no need to print
labels.

Let me know if you want help.
