I installed freemed at http://dhavaljani.com/freemed/ and when i login
using admin it shows only few menu items and when i try to do things
like add patient or view history or anything it says "access denied"
besides I dont know if it shows the full menu options as I dont see
options to create users etc...

Hi,
Congratulation on your successful installation.
There are many reasons why you may have permission problems.  The most common reason is that ACL has been turned on.  They don't work in this version and cause troubles.
Please go to Main -> Administration Menu -> System Configuration -> Access Control Lists.
Make sure that both of them say no.
Just to be sure, go to Main-> Administration Menu when you are admin.  You should see 6 categories:
Export Databases, Import Databases, Module Information, System Configuration, User Maintenance, CD/RW Backup
If you see User Maintenance, click on it and then click on Add to add a user to the system.
I also notice that your version is FreeMED v.0.84. It should be v0.8.5
Did you download FreeMED from SVN and install it on Debian as explained in:
http://groups.google.com/group/freemed-support/web/freemed-v-08-4-installation-instructions
If problems persist, consider contacting me offline at volker...@gmail.com
