double sided forms

Is there any way to use a double-sided form without having to do any sort of fancy programming?  I am designing a student evaluation form, and the set of questions runs over onto the back of the page, where there are also open ended text questions.  The form in its current configuration is attached.  It would be nice to be able to scan the forms so that page one is contiguous with page two and scored against the template for page one and then the template for page two.
