Sender field displays extra information and cannot reply

Hello, thanks for making this script. I have used it before with no problems but I'm having one issue. I have a form that people fill out and submit it. I then review the request and reply to the email approved or not with notes. So I need the sender field to either be the username or a field I created to capture emails. I did this (Picture attached) but when I get the form submitted (the email) it has the field as well as my email. If I try to reply it only send an email to myself. I need to take my email off and be able to reply to the person submitting. Thanks for your help.
