Updating FormEmailer

Hi Henrique,

I work for a charter school in California, and I am working on migrating your UiApp code to HTML templates so that we can continue using it.

I did not personally use this tool when it was functioning, and am wondering if you have any screenshots of the settings page UI? This would be a tremendous help, as reverse engineering Google UiApp code to HTML/JS/CSS without a point of reference is proving to take quite a while.

If you do not have any screenshots, I will still continue to work on this patch. If you would like, I can do a pull request when the updates are complete and FormEmailer is working again, so others can use it while you work on your full re-write

I was able to find an old tutorial video I made and got some low quality screenshots from it.
