Cant load Form Emailer Settings in my google sheet

Hello anyone have the same issue as me? See the attached picture...

Window with setting wont load.. and I donto know about HtmlService what is recommended to use in red stripe on the picture.

You have to update the Script.

The New version is Form Emailer 4.0 -- Link https://github.com/clyall/FormEmailer
