Issue with formemailer not following actual form

I changed my form to move the date and time fields on my actual form so that they would show up correctly in the spreadsheet. I then uninstalled and reinstalled the formemailer so that it could match the new form. However, now the email that is sent to me upon submission of a form has the date and time location in a different column (not in the order of the form). Also the date and time that is shown does not match the format selected on the form.

I am a casual user with limited html knowledge. 

Any thoughts?
