Continues to not work

The add-on is not functioning the way that it used to. It used to send out an email to an email address input by the user, but  now it only emails the administrator. Will this be fixed? Having it just email the administrator or even the user (through a collected email)  is NOT what I need it to do, and I will have to drop the add-on completely if it cannot be fixed to do what it used to do just a few months ago. 

This is extremely frustrating and disappointing.
