Add-on error

I updated the add-on and now am getting an error (see attached image) that says "Cannot call method indexof of null". The form will not email out properly - it sends the email to the administrator, but not the chosen recipient.

Please log out then log in your Google account and configure the add-on again. Did you try to re-install the add-on?

I have logged out of my account, uninstalled and re-installed the app. However, it still will not send the email to the address chosen. Please help.

Please revise, do you select the respondent's email field correctly? Was the add-on configured by one or multiple form's editors?

Yes, it is selected correctly. I am the only one who configures the email set up. I saw that others posted with similar problems. It is still not fixed -I just tried it. 

Another issue is, it used to let me collect the user's email, but now if I collect the user's email - it will not let me send the form to an alternate email (that is input into the form). It will just send it to the collected email. It used to allow me to choose who the emails would go to.
