comments on the f form:
1 lets try to understand from those people in banglore who claim that f form can genuinely tell them whether sex determination is done or no. ( which according to me is not possible by looking at the form. the only simple logic is about how many children they have and of what sex. this can raise the suspicion. still i may be wrong. i dont remember the exact name right now. but the organization at banglore which had claimed this and based on those observations the original f form was designed. some body in that area can be entrusted the job of understanding their view on the issue and give us the feedback.
2 the name of pt and age should be seperate columns so that they are filled up and " age forgotten to be written" shld not be a cause for " incomplete f form" and the sealing of machine and the drama later. simillarly the detials of the childrne shld have some lines blank there to write the details
3 since the most common reason for the usg is not going to be gentetic disease etc. points 9 and 10 should be restructured in such a way that the first option should be the most common option. even if it will fall in "others specify" category this category can be renamed . then should follow the uncommon things which are genetic disease metabolic disease etc. even we can put up more common indications such as growth, viability which are specified by the rules . so tht most often the person filling up the form will land up ticking rather than writing . normally if u r going to tell some one to write in " specify" column u will find tht it will be left blank most often!
4 regarding 14 point prenatal diagnostic procedure ( give details) : what detials are expected to be given here? . in half line in front availbale?
5 regarding point 15 and 16 . 16 deals with consent . how can consent come later to procedure in the form?
6 point no 17 need not be there at all. it adds to unnecessary writing. in fact the aim should be to reduce unnecessary columns
7 regarding MTP was done or no the last two points: there should be a specific mention asking " whether the doctor has performed the MTP or no?" rather than whether MTP was done or no . logically because the doctor can only know only if he has done or no he cant know if the pt has gone to some one else etc.
