Rendering multiple documents to a single PDF - handling internal links

Recently I had a problem when the document was to large to handle.

I decided to divide it to several sub documents that are merged to a single PDF file.

I found relevant example on the net and used it successfully.

The main problem was that internal links (between different sub-documents) stopped to function.

Does anybody have any idea how to handle this?

A smaller issue will be the page numbers
