Nested Categories / Subcategories on Custom Write Page

Having trouble with the Categories area on the Custom Write Page
showing all categories, nested or otherwise, in one long list, except
for the selected (checked) subcategories which do show up under their
main categories. I have 5 main categories and each has a few
subcategories, and I'd like the subcategories to show up beneath their
main categories.
I've already done the bug fix found here:

...that didn't help, and unselected (checked) subcategories remain in
a long list. Please help?
