question appears inside the freezed column

hello experts here, 

i encountered this problem twice. the question appears inside the freezed column. i have almost did everything like creating new sheet, trying it in different computer but there is no solution of it. It has happened twice.

Have you double-checked your setup to be sure the question isn't set as an identifying question?

That is what I think is happening.

If you still have a problem please share the spreadsheet and form with flubar...@edcode.org with edit rights. 

Dave or I will take a look.

Please include some detail about the problem so we can locate the problem quicker.
