Ultradox not triggering on Google Form submit

My Ultradox begins with a "Trigger by Form" block, but when I submit the associated form, the entire Ultradox doesn't appear to execute.

• I copied this UD from another of my Ultradox files, but changed all the associated Google resources to avoid conflicts (Form, Spreadsheet, Template, etc. are unique to the new project)
• I have several other UD's running just fine with the Trigger by Form block
• I tried deleting and re-adding a fresh "Trigger by Form" block
• When I run the project in Ultradox (by adding default values for the form), everything works perfectly.
• I've verified the form is submitting correctly, and populating the associated responses Google sheet, which is tied to the project.

Are there license limits on projects, or on how many forms you can trigger? I'm on the medium plan and only have 3 or 4 projects (all tied to Google Form triggers).

Did you make sure to activate the trigger in the Forms sidebar (Ultradox trigger)?

(slaps forehead) duh! I'm using new Google Forms and always forget to go back to old version temporarily to use the Add-On to activate trigger. Thanks, sorry for the newbie question!
