Looks like a problem with the FLARManager libraries or the version you are using.

In the tutorial Lee are using an ActionScript folder with all the required libraries, such FLARToolKit and FLARManager. Do you have these libraries in your project?

Of course you can. 

And yes. You could use any mp4 video. Also flv, mkv, mov and all the formats flash supports.

Eric has some examples, you can create a new Class and start with the Lee code.

I downloaded the flarmanager already,then i open a new actionscript project as what the tutorial ask to do. It still come out several errors at last of the tutorial :
