C. Obtaining this survey:
1. Send E-mail to the address above with "Survey Request" in the subject
line, and your E-mail address and inquiries in the body of the message.
2. Telephone or write. The Survey with a stamped (Japan only) addressed
envelope will be sent. Be sure to include a note stating your name and
address, printed clearly.
D. Privacy: All identifying information (such as name, E-mail, fax or
postal address) will be deleted, immediately. Under no circumstances will
identifying information be shared with anyone for any purpose.
E. Demographics: This survey is intended for adult, non Japanese women and
men, who grew to maturity in a country other than Japan, and who are
living, or have lived, in Japan for any length of time.
F. Instructions:
1. Dual (Male/Female; Yes/No) and Multiple (such as qualitative
assessments) Choice responses will be completed by deleting or "blacking
out" inappropriate choices, leaving only one (1) appropriate choice.
2. Multiple Answer responses (types of discrimination; reasons for leaving)
will be completed by deleting or "blacking out" inappropriate choices,
