Is there a format that's preferred?  

Make sure I'm repeating this correctly: people are directly purchasing the food  (free shipping) and getting the 7 Day Meltdown as the free bonus on the Thank You page?  So the order form only needs to show the DFY menu that they are purchasing?

I prefer the 7daymeltdown.com/fast-start/ format because of the guidance, each one is highlighted on the same page without having to click around, and the big, red arrows pointing to the order button.

Yes, I would imagine that as long as it's clearly stated in the sales copy that it comes free with purchase, it doesn't need to be on the order form.

Sara- You have that right.  People are directly purchasing food with free shippiung and we are throwing int he 7 Day as a bonus.    I think we put it all on the sales page.  We just need to update the copy a bit and use the fast-start format for purchases.
