When does Firebug display HTML in gray text?
For example, HTML hidden with CSS display:none. But also under other circumstances?
What triggers this? Are the conditions for this behavior listed somewhere?
In the screenshot you see HTML positioned off screen and thus invisible for a normal user. These elements are also grayed out.
Does that mean that Firebug is smart enough to understand this (off screen) is visually hidden?
But Firebug cannot differentiate between visually hidden (positioned off screen but visible for screenreaders and search engines) and completely hidden (display:none)?

Yes, at our wiki page for the HTML panel:

"Faintly displayed elements mean they are not visible inside the page. That is e.g. when the CSS style display: none is applied to the element or the element doesn't have any dimensions."

The logic for this is located in xml.js.
