Firebird 5.0.2 I/O error during "write" operation for file

Using fb 5.0.2 on ubuntu 22.04 LTS amd64 i keep getting
 
I/O error during "write" operation for file "/path/databse.fdb"
        Error while trying to write to file
        Input/output error

I'm trying to track down where and why but I'm a bit stuck

From what I can see there is plenty of disk space on the sdd
If I rerun some of the exact SQL then it works, it is very inconsistent

Explain how I can get more info for your debugging process., ie (logs, dumps etc)

There might be an defect in sdd, have a look at system logs or smartctl problem information.

I would check the kernel log (run "sudo dmesg") to see if there any
reports of file system or disk errors.

Followed advice, all smartd checked out ok, nothing to report from dmesg either. All other disk activity seems to be ok, backups and automatic restore testing work every day.
btrfs also reports no issues. It seems to be restricted just to Firebird.

I just went through all my lxc containers and recompiled the firebird.so for old legacy PHP.8.3-fpm
Also made sure that all clients are now running the latest version of the .so file
I calculated all the settings again in the firebird.conf and restarted everything, let's hope that this works.
