Message layout

Hello All,

in MessageNode::setup() I see following comment:

> // ASF: Odd indexes are the nullable flag.

It may be true for BLR generated from DSQL and ESQL but hand-written BLR can
use any layout because relationship between parameter value and NULL flag is set
in blr_parameter2 arbitrarily.
Is this method called only for auto-generated BLR?
