Affidavit of self-adjudication

Hello to everyone!

I recently had a patron approach me at the reference desk asking for a
sample of "affidavit of self-adjudication" -- she needs to submit this
to a bank in Manila. Since we are here in California, we have very
limited resources for finding Philippine legal forms, can somebody
please help me? We need the sample as soon as possible. Thank you.

Have you tried your Philippine consulate/embassy there?

They usually have these legal forms. Anyway, I'm sure this affidavit
will need to be certified or notarized by the Philippine
consulate/embassy there.

Tin,
I think any certified lawyer can help with an Affidavit of Self
Adjudication. Sabi sa akin dito i-draft lang daw yun personally, show
to lawyer / notary public, aayusin daw yun and have it notarized.
(wala yung ka-ofc namin na lawyer, e. sa secretary nya ko lang
natanung.)

Thank you to everybody!  Believe it or not, the embassy here in LA only has a few downloadable legal forms for its citizens and this particular form is not one of them.  Anyways, Aubrey (UPLSSA) got it for me through this website http://www.livinginthephilippines.com/form5r.html.  Funny thing was I was at this website yesterday and I could not find it -- oh no my searching capability has been compromised.  I've been telling my boss I need more training, alas to no avail.
