Subscription Form no longer working

I just found my subscription form I have on a page is no longer working and not sure what happened and how to fix it. When trying to subscribe I get an error message-see attached. My email subscriptions are active. I even tried turning it off and back on to try to see if that helped, but it does not. My subscription link I have on my site sidebar seems to be ok. Need some guidance to fix this. thanks

too weird. It all of sudden is working again.
