A Community Health Worker (CHW) on my team is currently supporting a young man diagnosed with Crohn’s disease. His physician has recommended supplemental nutrition shakes (such as Boost or Ensure) as part of his care plan. Unfortunately, his insurance no longer
covers these products, and the out-of-pocket cost has become a significant barrier for him. He has explored local food pantries but found that they do not always have a supply available.
We are exploring any available options to help bridge this gap and ensure he can continue following his prescribed nutrition plan. If your organization offers support in this area—or if you are aware of any community resources or programs that might be able
to help—I would greatly appreciate your guidance.
