How to delete this add-on

to Form Confirmation Emails - add-on for Google Forms
On the new Forms, let enter the Add-ons, navigate to the add-on that you want to remove, click it then manage and remove.
With the old Forms, let go to Add-on => Manage Add-ons, then remove the add-on you want.
