My Survey keeps getting blocked

My current survey keeps getting blocked for some reason.  I do not know how to stop it.  If I go in and edit the questions it appears that I can re-activate it,however, within a short period of time, the survey is again blocked.  This has been going on now for about two weeks.  I need help a.s.a.p.
