You need to add headings as per row no.2 & my formula is on blue colored cells, G45 & G57.

One more thing i want that option 1 was not catching in the old file. 

so i have change little bit & then revised it .. now you just modify formula so that i get desired result

I want that first product selection --->>> then group ---.>> then age ----> then from above box it calculate premium.

I have also add one more that if option 2 -- pop window have details as per option 1 selected.. 

You can understand better when u will see the wording in B57..

Once again thank a lot it will be really helpful me in otherfiles too ..

First of all make sure that you are giving exact name in drop down list  as in Table header to work formula correctly,

now to work dependent dropdown list to work, i have added data in sheet at the right corner in that first type plan header & below that add applicable lists & add define name for each header by selecting starting cell.
