Everything is correct. Dates are being skipped. What settings should I enable. Please guide me

This is browser default datepicker set date format like that - YYYY-MM-DD. Also check error in extension logs

Thank you. I changed to yyyy-mm-dd it worked 

The next problem is, after the form submitted and success page opens. It does not redirect back to the form page. What should I do. I copied success page URL and pasted it in success. Still did not work. Help

First check form URL and success form URL is same then set success form url in same url. If both url is different then insert a new form in extension with success form url then set success form settings in this form.
