How to create boilerplate-based documents based on form input?

I would like to create a document (e.g., a Statement of Work) based on a template or boilerplate doc that is tailored by inputting key fields (e.g., customer name) through a form.

I could do this as a Word template with a VBA form to generate the tailored document.

Can you recommend an approach to do this in GAS?

Both the UI interface and htmlservice are suitable for creating user interaction forms via GAS
