Ok. After more testing, I think it's safe to say that the problem is SOLVED. So here is the conclusion:
The problem:
-My GPU was overheated, to the point it exceeded the temperature limit, which is 83 C by default, so it throttle itself to remain under the heat limit after 15 minutes of gameplay. This is dueled to the fact that my GPU fans wasn't performed at full potential.
SOLUTION:
-Download MSI Afterburner, set the fan speed curve to a more aggressive percentage (100% at 70 C), and the GPU no longer throttle. Solid 60 fps during my 2 hours of testing.
I post this here so if anyone's having the same issue, here is a throughout solution. Kudo to the Nexus member AtomicFallout757 of the Buffout 4 community, who pointed out the GPU heat issue right away, just by seeing my before and during pictures of the lag. Seriously, thank you kind sir. You are the light in the deepest dark for me and so many people. I can't thank you enough. And that's it. See you guys out there in the waste, fellow wanderers.
My game start out real smooth (solid 60 fps in my settlement Sanctuary), then becomes very laggy after 10 to 15 minutes of playing (30 fps in the same location). I think I have a memory leak, which is a totally new concept to me. My game is modded (218 plugins total, more info below), and I have some modding knowledges, specifically: load order, BethINI tweaking, using the mod Buffout 4, and FO4Edit. I have basic knowledge about PC (been using it my whole life), and unfortunately: zero about codding, computer mechanics,... You know, the good stuff, so i have no clue how to fix this or even where to start. Any help will be tremendously appreciated, and I mean ANY. Point me to a direction and I'll chase it, whatever it cost. I love this game too much, and I can't live without it (long story short: it's basically my life), or at least not having to play for 15 minutes, save, quit the game, then load it up again. If I have a code for this situation, it will be code RED, since it freak me the F out rn.
Anyway, enough of me whining, here is some more infos for you guys who want to help:

Might try opening up performance monitor, leave it running, play for a bit, and when the game starts to lag, exit out, and have a look at the graphs, to see where the basic problem is. (filling up ram, maxing the cpu, hdd activity, etc. I *think* win 10 pro will also monitor GPU memory as well.)
I feel your pain Have you considered that maybe your GPU is throttling due to heat issues? I have been having lagging issues and I have pretty much tried everything that have been recommended, including Throttlestop to reduce voltage going to the CPU and MSI Afterburner to basically under-clock my GPU to reduce it from getting too hot. Throttlestop has helped with my CPU as it is working within specs and the temperature is great, but after I tried under-clocking my GPU, it actually ran hotter, which makes no difference unless there is an issue with the GPU itself. A slight overclock to the GPU gave me much better performance, but the throttling was still there due to heat issues, at least, that is what I believe is happening as I have tried all software related tweaks and none of them are working. My issue is primarily due to the fact that I use a laptop, it is not that old (less than 2 years old) but it may need some maintenance such as re-pasting the GPU with maybe liquid metal.
I do not see issues with your mods (I am not an expert) but the fact that you are reporting stuttering and not constant CTDs tells me it may be hardware since you have stated that you have tried many of the performance mods.
The error in your Buffout references your N-Vidia card. The MO suggests downgrading the driver to an older version, but I am not fully convinced the driver is the issue but merely a symptom of overheating. I have 2 fans and the laptop fans at max and my video card is still hitting in excess of 80C.
Another possibility is having a large amount of texture files, such files can slow down the GPU. I have eliminated about 25 texture files, that did nothing. I reduced the graphics quality from good to low, nothing. These things however, may help you.
Finally, some weather mods can have an impact on stability. I have found while in game my stuttering increases dramatically when there is a heavy rad storm. When I change it ti clear weather, the stuttering is substantially reduced or is non-existent.
I wish there was a silver bullet I could point you to, if there is, I have not found one. I really enjoy this game as I find it relaxing but dropping down to 18 FPS and then jumping back up to 60 is simply intolerable. In monitoring the GPU, I see that it sometimes drops down to 300 mhz, WAY too low, and then jumps up to 1300mhz. Since it is a laptop, I cannot really upgrade the GPU with anything faster as the heat issue will continue to occur.
I wish you the best of luck and will keep my eye open on this topic (if I can figure out how to get notifications). if I come up with any ideas. I will share those with you.

In conclusion: I don't think it's the CPU's heat my man. The cores are nice and cool (all within 50 Celsius, which from what I heard is amazing). And ignore the FPS counter, it's very high when I alt-tab out of the game.

-I have ruled that possibility out quite early, by switching between weathers rapidly (my weather mod NAC X have weather loading features), and lo and behold, solid 60 FPS on every type of weather. I don't think it's the problem my man.
Like I said, the only abnormal thing I have noticed is the fact that my hard drive "active time" and "disk transfer rate" (read and write speed) are near to non-existing when the game started to lag, according to the Task Manager performance graphs. Something is preventing the disk to work properly, or lower the disk reading capacity, I don't know, I can only read what the graphs showed me. Can you think of a reason for this kind of shenanigans, because it is super weird. I haven't seen or heard of anything like this before.
4 Terrabyte HDD?? I suspect that may be part of your problem..... Generally, the bigger the drive, the slower they are. Unless it's a 15000 RPM drive..... I have also seen that kind of behavior on drives that the cache is having issues..... One of the reasons I went to solid state drives for my games.
The hard drive may be the culprit if you are not seeing the proper read and write states. I assume that they are fairly new? I use SSDs so read and write is usually not my problem. It is heat that is killing my system for now. I saw the temps on your CPU and they are good. Mine are also good,\but what about the GPU?
The old HDD is a Seagate Barracuda 4TB (2017), switching to a HDD WD Black 1TB (2013). According to the Task Manager's graph, my CPU is running at 93% at max. The FPS normally always at solid 60, but after 15 minutes, it dunked to 30 and can never be recovered, no matter what I do. The only way to fix this is save, close the game and reboot it.
So I've been playing fallout NVUE for about a week straight no life-ing the shit outta it but all of a sudden im getting such a massive lag out of no were and when the lag isn't there my game freezes randomly for no reason its not the disc i bought it new and its starting to urk me to the point i have to keep getting up to restart my ps3 every 10 minutes Is there a fix to this?
Also to note the whole week I've played i haven't had this issue it just recently started today idk why.
Turn autosave off, clear the cache (L2, R2 and Square simultaneously as the game is loading) and make new saves whenever you need to save is my best advice. It won't eliminate it completely but it should reduce it a little.
I don't think it does, I forgot to mention that you need to press it after the anti-save file corruption message and trophy loading messages have disappeared and then hold until you reach the loading screen.
The game is a technical mess. It gets worse the larger your save file is. Good luck getting through all the DLC on one file. I did. But I saved Lonesome Road for last and the first part of it was almost literally unplayable. It's a great game and I spent a lot of time with it, but there's not much you can do. Some areas are more prone to lag/freeze, I would say definitely in and around New Vegas itself, or really any of the more populated areas.
TBH i had no problems intill lately and the freezing and lag stopped after i took a pistol out of my inventory and it lessened the lag it was 22. Silenced or something and i already beat lonesome road without and lags besides the rocket launching that froze once but it was all good.
